1. Description of the Organization (maximum 5 points, please limit to ½ pages)

The Wings Home has been an irreplaceable asset to the community since it started caring for dying individuals in October of 2010. This innovative non-profit organization is the only hospice-founded end of life care home in Michigan and the only specialized place in Allegan County where dying individuals can live if they cannot stay in their own homes. Our ranch-style 4-bedroom home is available to any Wings of Hope Hospice patient who needs a place to live during their last few weeks of life. These individuals either have nowhere else to go, have no one to care for them or have loved ones who can no longer care for them. There is no cost to the patient to stay at the Wings Home. Located in Allegan, Michigan, the Wings Home serves individuals from Allegan, Barry, Kalamazoo and Van Buren Counties and the surrounding communities. Since 2010 the Wings Home has served more than 1,000 patients. Staff and volunteer caregivers provide around-the-clock care for our patients.

We believe that everyone, regardless of their financial/social/emotional situation, sexual orientation or gender identity deserves compassionate care at the end of life, the minimizing of suffering and a death that is as comfortable as possible. Individuals served by Wings of Hope Hospice who may be in their last 6 weeks of life are eligible to stay at the Wings Home.

2. Funding Request (maximum 35 points, total, please limit to 1 page)

Funding is being requested from West Shore Aware for Wings Home caregiver wages which make up 72% of the organization’s budget. The sole purpose of the Wings Home is to provide care to dying individuals. This includes all the care that a family would provide a dying loved one: meal preparation, changing briefs, helping with medication, cleaning, laundry and providing emotional support to the patient as well as the patient’s family.
Our objectives are to provide good care in a safe environment with the privacy needed by the patient. Our caregivers strive to be helpful and welcoming to all patients and their family or other visitors. If families are willing to recommend Wings Home to others, we believe we have achieved our objectives.
A satisfaction survey is sent to each patient's family 30 days after the death of their loved one. With a 37% return rate, these are the 2023 survey results:

West Shore Aware funds would be used to support caregiver wages which helps the Wings Home give dying patients and their families what they need during a critical time.

3. Funding Justification (maximum 40 points, please limit to 1 page)

Funding from West Shore Aware would pay for part of the project. Funds are also requested from Allegan County Community Foundation, United Way, Mary Thompson Foundation, Delano Foundation, Perrigo Foundation and donations from the community.
There is no expense to the patient to stay at the Home. The Home receives no government or insurance funding but instead runs entirely on community support. This includes donations (41%), golf scramble (18%), events by others (6%) and grants such as offered by West Shore Aware (35%).
In 2023, 21% of the care given to patients was provided by volunteers. This is valued at $40,857.
The Wings Home will succeed through the continuing recruitment and training of volunteers to provide caregiving. We will continue to fundraise including pursuing grants such as this one to pay for staffing and other needs. We are committed to providing quality care for our patients. There is no other option than to succeed in obtaining funding.

4. Summary

A dying individual needs increasing amounts of hands-on care as they near the end of their life. This can be a challenge if family lives far away or if they are exhausted caregivers or in frail health themselves. Options for a place to live include several institutional facilities that charge $250 to $400 per day. Located in Allegan, Wings Home is a ranch-style 4-bed home that provides care to Wings of Hope patients in their last weeks of life at absolutely no cost to the patient or family. Since 2010, the Wings Home has cared for more than 1,000 individuals. Everyone, regardless of their financial/social/emotional situation, sexual orientation or gender identity, deserves compassionate care at the end of life without worrying about how to pay for it.
Staff caregiving at the Home is estimated to cost $193,709 in 2024. Volunteer caregivers provided 21% of the care in 2023.
Receiving no government/insurance money, the Wings Home exists entirely with the support of the community. We are requesting $10,000 from West Shore Aware. We estimate this would support caregiving for 4 patients for 16.6 days, a value of $16,600 to $26,560. Thank you for the opportunity to make this request.

Application AgreementThis is an application, otherwise known as a Request for Funding (RFF), from West Shore Aware. This RFF is a request from your organization to Aware, for Aware to host a Fundraising Event for your organization. By completing this RFF, your organization is requesting to be considered by Aware’s membership for an Aware fundraising event to be held during the summer of this year, also known as the White Party.

If your organization is chosen for a fundraising event by the Aware membership, your organization will agree to the following terms and conditions for the said fundraising event.

1. Aware will be given and provided full credit for the fundraising event in all of you organization’s publicity and advertising for said event, to include but not limited to event posters, newspaper advertising, handouts, radio and television advertising for the event, and organizational announcements concerning the event.

2. Your organization will only use the funds raised by Aware from said event for the purpose described in the RFF submitted to Aware.

3. Your organization understands that West Shore Aware is a philanthropic organization of LBGTQ+ individuals and their supporters, and that the funds your organization receives from Aware will be coming from the West Michigan lakeshore community.

4. The funds received from Aware as a result of the fundraising event will be net, after expenses for said fundraising event are paid.

5. Aware may use the name of your organization in its own advertising and publicity as a recipient of funds..

6. Your organization will supply Aware with written approval to hold, or be part of a fundraising event held by Aware.

7. West Shore Aware reserves the right to audit any organization that receives funding to determine whether the funds were used as described in its RFF.

8. Your organization will allow West Shore Aware to use you organization’s name on the West Shore Aware website. In addition, if your organization has a website, you will link that website to the West Shore Aware website and allow West Shore Aware to link to your organization’s site. West Shore Aware’s web site link must remain on your web site for a full year from the date of the monetary disbursement. Failure to do so will disqualify you from requesting future funding.
