1. Description of the Organization (maximum 5 points, please limit to ½ pages) | Wings of Hope serves individuals and their families in Allegan, Barry, Kalamazoo and Van Buren Counties and portions of Ottawa, Kent, Ionia, Calhoun, St. Joseph and Cass Counties. Our mission is to “provide compassionate care at the end of life and support through the grief experience for individuals and families.” This includes chosen families and relatives. We believe that everyone, regardless of their financial/social/emotional situation, sexual orientation or gender identity deserves compassionate care at the end of life, the minimizing of suffering and a death that is as comfortable and dignified as possible. We serve people wherever they live: in their homes, in nursing homes and at the Wings Home. This care is provided with no out-of-pocket cost to the patient or their family or caregiver. Our target population are individuals with a terminal illness and a life expectancy of 6 months or less who are no longer seeking curative treatment. We primarily serve adults but have also served children and babies. Most of our patients are over the age of 65 with all types of illnesses including cancer, dementia, heart disease, lung disease, liver and kidney disease. We meet the needs of this target population by managing pain and symptoms in hopes that they can experience their definition of quality of life for as long as they live. We also serve the patients’ chosen families and relatives, friends and caregivers at all stages of this journey, including grief support. Wings of Hope has been serving the community for 40 years and enjoys tremendous community support. We believe this is because our standards go beyond what is legally required of us. We provide excellent, ethical care because it is the right thing to do. |
2. Funding Request (maximum 35 points, total, please limit to 1 page) | Wings of Hope’s Pain & Symptom management program serves individuals with a terminal illness and a life expectancy of 6 months or less. The program benefits them as it helps them cope with pain and symptoms that accompany these illnesses. A hospice patient will experience many changes, physically, emotionally, cognitively, socially and spiritually. Pain and symptom management is accomplished through the use of medications, oxygen, medical equipment and supplies especially tailored to the needs of each patient. The hope is that they can experience quality of life during their last days. Wings of Hope also helps to manage depression, stress, and anxiety at the end of life because these can exacerbate pain and worsen quality of life. Studies have shown that individuals who are older and who identify as LGBTQ+ have depression at a rate four times that of the wider population. Quality care is provided with the combined teamwork of the skilled professional services of our clinical team. Patients and their loved ones receive support for emotional, social and spiritual issues from social workers and spiritual care coordinators. Wings of Hope also provides care for patients and their families who are uninsured and underinsured. The cost of skilled professionals, including a Medical Director, Medical Records, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, Social Workers, Spiritual Care Coordinators, and Volunteer services are not included in this grant request. The estimated cost in 2023 for medications is $178,451.80, oxygen and equipment, $143,700.50 and medical supplies, $29,926.48. The cost of hospice care may also include hospital stays to manage symptoms that cannot be controlled at home, respite stays to give the caregiver a much-needed break, lab tests, therapies, x-rays, and port insertion to drain excess body fluid. Wings of Hope’s goal is to continue to provide quality care through pain and symptom management for hospice patients. In 2022, Wings of Hope served 329 patients and we estimate we will serve 330 patients in 2023. Wings of Hope is requesting a grant of $10,000 from West Shore AWARE to assist in covering the costs of medications, oxygen and equipment and medical supplies. Your support is deeply appreciated. |
3. Funding Justification (maximum 40 points, please limit to 1 page) | The funding requested from West Shore Aware will pay for a portion of the services provided to hospice patients. The remaining funding will be obtained through other grants, individual donations, events by others and special fundraising events such as our annual Night of Hope auction held in May. We anticipate using all of the funding as our 2023 budget for medications, oxygen and equipment and medical supplies alone are a total of $352,078.78. Operating funds come from billing Medicare, Medicaid and private insurance for services rendered. Zero percent of the organization’s operating funds come from government grants or private grants such as those from West Shore Aware. Our Pain & Symptom management program is an integral part of our services and the program will continue, as there is no other option. Our goal is for every patient we serve to have the pain and symptom management they need so that they have the best quality of life possible in their last months, weeks and days. This benefits them as well as their loved ones. |

Application Agreement | This is an application, otherwise known as a Request for Funding (RFF), from
West Shore Aware. This RFF is a request from your organization to Aware, for Aware to host a
Fundraising Event for your organization. By completing this RFF, your organization is requesting to be considered by Aware’s membership for an Aware fundraising event to be held during the summer of this year, also known as the White Party.
If your organization is chosen for a fundraising event by the Aware membership, your organization will agree to the following terms and conditions for the said fundraising event. 1. Aware will be given and provided full credit for the fundraising event in all of you organization’s publicity and advertising for said event, to include but not limited to event posters, newspaper advertising, handouts, radio and television advertising for the event, and organizational announcements concerning the event. 2. Your organization will only use the funds raised by Aware from said event for the purpose described in the RFF submitted to Aware. 3. Your organization understands that West Shore Aware is a philanthropic organization of LBGTQ+ individuals and their supporters, and that the funds your organization receives from Aware will be coming from the West Michigan lakeshore community. 4. The funds received from Aware as a result of the fundraising event will be net, after expenses for said fundraising event are paid. 5. Aware may use the name of your organization in its own advertising and publicity as a recipient of funds.. 6. Your organization will supply Aware with written approval to hold, or be part of a fundraising event held by Aware. 7. West Shore Aware reserves the right to audit any organization that receives funding to determine whether the funds were used as described in its RFF. 8. Your organization will allow West Shore Aware to use you organization’s name on the West Shore Aware website. In addition, if your organization has a website, you will link that website to the West Shore Aware website and allow West Shore Aware to link to your organization’s site. West Shore Aware’s web site link must remain on your web site for a full year from the date of the monetary disbursement. Failure to do so will disqualify you from requesting future funding. Once you have completed the RFF and agreed to the aforementioned, please submit it. Please sign this document below, indicating acceptance of the aforementioned terms and conditions. By signing the document, you acknowledge your organization is a registered 501(C) 3 Charitable organization. |
